| Liability Release: Contestant agrees to participate in JEND PRODUCTIONS sanctioned events at Contestant's own risk. Contestant hereby releases and discharges the JEND PRODUCTIONS and its directors, officers, representatives, employees and agents from all liability, loss, claims, damages and expenses for injuries to person, property, reputation or financial condition as a result of or in any way relating to Contestant's participation or failure to participate in any JEND PRODUCTIONS sanctioned events, whether caused by negligence, by arena or facility conditions, by the conduct of the JEND PRODUCTIONS or its directors, officers, representatives, employees and/or agents in the preparation, supervision or conduct of any JEND PRODUCTIONS sanctioned events or the administration or failure to enforce any JEND PRODUCTIONS rules, regulations or guidelines, or otherwise. |
| Contestant knows and agrees that by his or her signature on this form, he or she completely releases the JEND PRODUCTIONS and its directors, officers, representatives, employees and agents for any liability, including negligence. Contestant voluntarily chooses to participate in JEND PRODUCTIONS sanctioned events and freely and willingly consents to same. Contestant further acknowledges that he or she has no absolute property or other right to participate in JEND PRODUCTIONS events. Contestant agrees to follow and be bound by the rules, regulations, and guidelines of the JEND PRODUCTIONS as amended from time to time. Contestant agrees that his or her sole and exclusive remedy for any disputes is appealed to the JEND PRODUCTIONS appeals board pursuant to JEND PRODUCTIONS appeal procedures, and agrees that all decisions of the appeals review group are final and conclusive. |
